Clay Body Approval Form

Any clay that is not purchased through the Clay Lab must be approved by the Clay Lab Manager prior to it being brought into the studio.

To obtain approval, please fill out the following form.

After submitting, you will receive an approval response from the Clay Lab Manager via email. Written approval via email must be obtained before the clay is brought into the studio.

Note: If you are attempting to obtain approval for multiple types of clay, please fill out a separate form for each individual clay body.

Please provide a link to the clay body on the website you wish to purchase from.

Please provide a link to the clay manufacturer's website if not purchasing directly from the manufacturer.

This is needed in cases when you intend to purchase from a distributer such as Blick who does not produce the clay body themselves.
